Connect to... is Derbyshire Voluntary Action’s new enhanced social prescribing and self-care service to support people who are lonely, isolated or at risk of developing/living with, a long-term health condition. This service will help local people increase their social and physical activity, and access emotional and practical support.

The service will enable GPs and other health and care professionals to connect their patients to a range of non-clinical and community support services.

The Connect to... team will include Community Navigators who will be out and about meeting people in GP practices community hubs, and venues. They will be able to visit clients in their homes and offer holistic support that may span over a few months. When appropriate they will work with clients to develop a goal setting action plan.  Key to this service will be volunteer Social Connecters who will be local people with a desire to help others and to make their community a more caring and connected place to live.  A Community Coordinator and a Support and Outcomes Officer will bring the service to life and support the volunteers.

Connect to... will initially be available across Chesterfield and will work as part of an integrated health and care system working to improve people’s social connectivity, health and wellbeing.

We hope it will prove to be a great new model of care and subsequently roll out across North East Derbyshire and Bolsover.
